Confidentiality and Privacy Policy
Next Steps Housing is committed to maintaining the confidentiality and privacy of all residents in accordance with 42 CFR Part 2 and Indiana state laws regarding substance use disorder treatment records.
Confidentiality Protections
- Protection of Resident Information
- All records and communications related to a resident’s participation in the program are confidential.
- Disclosure of resident information is strictly prohibited without written consent, except as permitted under federal and state law.
- Consent for Disclosure
- Residents must sign a written consent form before any information can be shared with external parties, including family members, employers, or legal representatives.
- Consent may be revoked at any time in writing by the resident.
- Exceptions to Confidentiality
- Disclosure is permitted without consent in the following cases:
- Medical emergencies.
- Reports of suspected child abuse or neglect.
- Court orders that comply with federal and state regulations.
- Threats of harm to self or others.
- Disclosure is permitted without consent in the following cases:
- Secure Record-Keeping
- Resident files and records are securely stored and accessible only to authorized personnel.
- Electronic records, if applicable, are maintained in compliance with HIPAA and 42 CFR Part 2 security standards.
